How Texas organizes public mental health care

The Texas Health and Human Services Commission (HHSC) contracts with 37 Local Mental Health Authorities (LMHAs) and two Local Behavioral Health Authorities (LBHAs). Each one covers a set of counties, assesses the mental health needs of its area, plans and coordinates services, and runs a 24/7 crisis line.

Most Texans with Medicaid get coverage through STAR managed care, and children's Medicaid, including behavioural health, is delivered mainly through health plans.

Built Around How Texas Care Teams Work

  • Texas privacy law, Medicaid managed care and the state's licensing boards each shape how outcome data is collected and kept. HiBoop handles the measurement side and leaves the clinical call with you.

  • HIPAA, With Chapter 181 in View

Texas Health and Safety Code chapter 181 reaches further than HIPAA in who it covers, and adds staff training and electronic-disclosure duties. HiBoop signs a BAA, is SOC 2 Type II certified and keeps US customer data in Google Cloud US regions, so the vendor side of your file is in order.

  • Session-by-Session Tracking

Send the PHQ-9, GAD-7, PCL-5 or C-SSRS Screener before each session. Scores arrive totalled with severity bands and land on one chart per client, so change across a course of treatment informs your next step.

  • Collaborative Care in Primary Care

Texas Medicaid lists collaborative care model services among its behavioural health benefits, after S.B. 672 (2021) required HHSC to pay for them. Care managers can run repeat PHQ-9 and GAD-7 check-ins for a primary care caseload and bring the trend to the psychiatric consultant.

  • Clinics, Group Practices and Community Providers

Team and Network plans give a group practice, clinic or multi-site provider one place for every clinician's caseload, with role-based access so staff see only what their role allows.

  • One Published Price List

Care is $25/month for one clinician, month to month. Team starts at $200/month with unlimited clinicians. Network is custom. One price list in US dollars, published on our pricing page.

  • Records Clients Can Take With Them

Chapter 181 gives clients a right to their electronic health record within 15 business days of a written request. HiBoop exports a client's score history as a PDF or CSV, so the outcome part of that request is a few clicks.

Research from Texas

  • Measurement-based care for youth

A chart review across Texas Youth Depression and Suicide Research Network sites found standardized depression measures in 71.2% and anxiety measures in 64% of reviewed visits, while standardized measures of medication adherence and side effects were each used in under 6%.

Slater et al., Child Psychiatry Hum Dev, 2025 PMID 38340213

  • Measurement-based care in primary care

In the VitalSign6 project led from UT Southwestern in Dallas, 17.3% of the first 25,000 patients screened in 16 primary care clinics screened positive for depression, and remission among those started on measurement-based pharmacotherapy was 20.3%, 31.6% and 41.7% with 1, 2, and 3 or more follow-up visits.

Jha et al., Ann Fam Med, 2019 PMID 31285210

  • Depression screening

Among 11,803 uninsured patients seen in nine community-based clinics, in a study by UT Southwestern and other Texas researchers, 67% had a depression screener recorded and 24% had a depression diagnosis, but more than half of those diagnosed had no recorded treatment plan.

Rezaeizadeh et al., Int J Clin Health Psychol, 2021 PMID 33981346

  • PHQ-9 in routine care

In 116 veterans with spinal cord injury or dysfunction who completed the PHQ-2 and PHQ-9 at a routine annual evaluation, in a study from Houston's Michael E. DeBakey VA Medical Center and Baylor College of Medicine, a PHQ-2 cut-off of 3 had 83.3% sensitivity and 97.8% specificity for a PHQ-9 score of 10 or more.

Poritz et al., J Spinal Cord Med, 2018 PMID 28355958

Where Measurement Fits in the Texas System

  • HiBoop is independent of these programs and does not connect to their systems. This is the landscape your clients, referrers and contracts already sit in.

  • Local Mental Health and Behavioral Health Authorities

    HHSC

HHSC contracts with 37 LMHAs and two LBHAs, each responsible for planning and coordinating mental health services in its counties and for providing treatment and referrals.

Texas Medical Records Privacy and Your Outcome Data

  • Chapter 181 of the Texas Health and Safety Code sits on top of HIPAA. Its definition of a covered entity is wider than HIPAA's, reaching anyone who collects, stores or transmits protected health information, and it adds duties of its own. There is no certification for it; the duties sit with each covered entity, and the right vendor makes them easier to meet.

  • What Chapter 181 asks of covered entities

    • Train staff on state and federal health privacy law within 90 days of hire, and again within a year of a material change in that law (s.181.101)
    • Keep each employee's signed training statement for six years (s.181.101(d))
    • Give a client their electronic health record within 15 business days of a written request, where your system can produce it (s.181.102)
    • Post notice that health information may be disclosed electronically, and get authorization for each electronic disclosure outside treatment, payment and health care operations (s.181.154)
  • What HiBoop does

    • Signs a Business Associate Agreement and complies with HIPAA
    • Hosts US customer data on Google Cloud in the US, encrypted in transit and at rest
    • Logs each view and export of client data, with role-based access controls
    • Is SOC 2 Type II certified, with the report available on request
  • Need a BAA or our security documentation?

Texas practices often ask a vendor for its security controls and a BAA before signing. Our trust page covers hosting, encryption, access controls and SOC 2 Type II.

Frequently Asked Questions

  • Does HiBoop meet the Texas Medical Records Privacy Act?

There is no certification under chapter 181 of the Texas Health and Safety Code. Its duties, such as staff training within 90 days of hire, signed training records kept for six years and notice of electronic disclosure, sit with each covered entity, and the Act's definition is wide enough to reach practices and vendors alike. HiBoop's part is the vendor side, set out control by control on our trust page.

  • Which Texas professionals is HiBoop built for?

Licensed Professional Counselors (LPCs), licensed marriage and family therapists, psychologists and social workers, whose boards sit under the Behavioral Health Executive Council, plus the psychiatrists, primary care physicians and physician assistants licensed by the Texas Medical Board. Clinics, group practices and community providers use it to run check-ins across a caseload.

  • Can providers in Texas Medicaid plans use HiBoop?

Yes. HiBoop is used by individual clinicians and organizations, not billed to Medicaid, so it sits alongside whichever STAR, STAR+PLUS, STAR Kids or STAR Health plans you contract with. It does not submit claims or connect to plan systems. Check each plan contract for the specific measures it names.

  • Does HiBoop work with LMHAs or Texas CCBHCs?

HiBoop is independent of HHSC and does not connect to state systems. Local authorities, certified clinics and the providers that work with them can use it to send validated self-report scales such as the PHQ-9, GAD-7, PCL-5 and C-SSRS Screener and see each client's trend. Any state-required intake or level-of-care instrument is outside HiBoop's library and stays where you run it today.
