Measurement-Based Care for Illinois Mental Health Teams

Illinois Medicaid already pays for collaborative care that repeats a validated scale every month. HiBoop sends the PHQ-9, GAD-7 and other scales by email or QR code, scores them as clients submit, and keeps a running chart per client for primary care clinics, CCBHCs and practices from Rockford to the Metro East. Data for US customers is hosted in Google Cloud US regions.

Illinois Medicaid Already Pays for Measurement

  • Since 1 July 2022, the Illinois Department of Healthcare and Family Services (HFS) has covered the Psychiatric Collaborative Care Model in both fee-for-service Medicaid and HealthChoice Illinois managed care, under Public Act 101-0574. The HFS guidelines spell out what that care looks like: an initial assessment that includes at least one validated measurement tool, with the PHQ-9 and GAD-7 given as examples, a registry kept by the behavioral health care manager, and the same tool given again every month to track progress. Practices attest that they follow the model when they enrol and again each year by 1 October.

Community behavioural health works the same way at a different level. HFS designates the IM+CANS as the approved tool for Integrated Assessment and Treatment Planning: a provider must use it to be paid for that service, and must review and update each plan at least every 180 days. The IM+CANS also decides eligibility for Pathways to Success, the Medicaid program for children and young people under 21 who need intensive home and community-based support.

HiBoop does not offer the IM+CANS and does not file anything with HFS. What it covers is the symptom side: the monthly PHQ-9 or GAD-7 a collaborative care registry needs, and the repeat scores a clinician wants between six-monthly IM+CANS reviews, each scored on submission and charted per client.

  • A Validated Scale Every Month

HFS collaborative care guidelines, covered since July 2022 with an annual attestation due each October

  • IM+CANS Every 180 Days

The approved assessment for IATP, required for payment in community behavioural health

  • 19 CCBHC Sites

Serving Illinois clients since October 2024 under the federal CCBHC demonstration

How HiBoop Fits Illinois Programs

  • Illinois ties Medicaid behavioural health payment to structured assessment and repeated measurement, and has its own rules on records and on young clients' consent. HiBoop does the sending, scoring and charting; your clinicians decide what the numbers mean.

  • A Registry That Fills Itself

Schedule the PHQ-9 or GAD-7 to go out monthly to every client on a collaborative care caseload. Each completed form is scored on arrival and lands on that client's chart, ready for the psychiatric consultant's caseload review.

  • Between IM+CANS Reviews

An IM+CANS update comes around every 180 days. Repeat symptom scales in the months between show the direction of travel early, so a plan review starts from data rather than recollection.

  • Your Records, Exported

Download a client's score history as PDF or CSV for a chart audit, an MCO request or your own quality work. Account-wide exports are there whenever you need them.

  • Anxiety, Depression and Suicide Risk Together

The PHQ-9, GAD-7 and C-SSRS Screener sit side by side, so a primary care team can screen for the two conditions collaborative care most often treats and follow a positive item 9 with a structured risk screen.

  • Scales Sized for Young Clients

Illinois lets a young person aged 12 or older start outpatient counselling on their own consent. For school-age and adolescent caseloads, HiBoop offers the PHQ-A, SCARED and NICHQ Vanderbilt.

  • One Account for a Whole Network

A CCBHC or multi-site agency can put every clinician on the Team or Network plan, with role-based permissions deciding which staff see which caseloads.

  • 21.5%

of Illinois adults received mental health treatment in the past year (2024)

  • 11.1%

Research from Illinois

  • PHQ-2 and PHQ-9 in collaborative care

Across 10 primary care clinics in Chicago, 25,369 people were screened with the PHQ-2 and PHQ-9 between November 2016 and December 2017; screening, referral and uptake were proportionate across subgroups except adolescents aged 12 to 17, whose age was associated with disproportionate PHQ-9 screening and treatment disengagement.

Thompson et al., Psychiatric Services, 2019 PMID 31023189

  • Perinatal collaborative care

In five obstetric clinics delivering at one Chicago hospital, 7,028 women were studied before and after the COMPASS perinatal collaborative care program began in 2017; antenatal depression screening rose from 33% to 81%, and treatment recommendations after a positive screen from 44% to 61%.

Miller et al., Psychiatric Services, 2021 PMID 34015950

  • PHQ-9 and GAD-7 targets

At a nurse-managed health centre in an underserved Chicago community, 166 people received collaborative care in its first year, with goals of a PHQ-9 or GAD-7 score below 5 or a 50% reduction; 22% reached the depression goal and 47% the anxiety goal.

Reising et al., Journal of the American Psychiatric Nurses Association, 2023 PMID 34431726

  • Measurement-based care with a digital tool

In Illinois primary care clinics, adults starting or switching an antidepressant who used a digital care pathway (89) were more likely than a pre-implementation control cohort (90) to complete at least two PHQ-9s in 6 months, 19% versus 12%.

Khatib et al., JMIR Formative Research, 2026 PMID 42562382

  • Perinatal depression screening law

A systematic search of the Illinois General Assembly found two major Acts on perinatal mental health, one requiring universal perinatal depression screening and one raising awareness of maternal mental health symptoms and treatment.

Illinois Programs That Shape Measurement

  • HiBoop is not part of these programs and does not connect to HFS, IDHS or MCO systems. They are the context an Illinois clinic director will already be working within.

  • Psychiatric Collaborative Care Model

    Covered since 2022

HFS covers 99492, 99493 and 99494, and G0512 for FQHCs, rural health clinics and encounter-rate clinics, in fee-for-service and managed care. The guidelines require a validated measurement tool at the first assessment and monthly after that, a registry, regular caseload review with a psychiatric consultant, and an attestation renewed every October.

Illinois Rules on Mental Health Records

  • Alongside HIPAA, Illinois has a separate statute for mental health records, the Mental Health and Developmental Disabilities Confidentiality Act, with its own rules on consent and access. The duties below fall on your organization; no software certifies them, and HiBoop's role is to give you the controls you need.

  • What Illinois law expects of you

    • Disclose records only with written consent that names who receives them, why, what is shared, the date the consent expires and the right to revoke it; blanket consent to unspecified information is not valid (740 ILCS 110/5)
    • Do not let a recipient redisclose what you sent unless the person who consented specifically allowed it
    • Let a client aged 12 or older inspect and copy their own record; a parent of a 12 to 17 year old has access only if the young person is told and does not object, or the therapist finds no compelling reason to refuse (740 ILCS 110/4)
    • Keep HIPAA's safeguards and breach duties, with a business associate agreement for any vendor that holds protected health information
  • What HiBoop gives you

    • A signed BAA before any client data enters your account
    • Hosting for US accounts in Google Cloud US regions, with encryption at rest and in transit

Measurement-based care

…starts with what we measure, and how long.

Frequently Asked Questions

  • Does Illinois require measurement-based care?

Not across the board, but two Medicaid services build it in. Collaborative care billed to HFS must include a validated measurement tool at intake and every month after, and community behavioural health providers must complete the IM+CANS for Integrated Assessment and Treatment Planning, reviewed at least every 180 days. Outside those services we found no Illinois statute that requires routine symptom scales.

  • Can HiBoop replace the IM+CANS?

No. The IM+CANS is the HFS-approved tool for IATP, completed by certified staff, and HiBoop does not offer it. Teams use HiBoop alongside it, for short self-report scales such as the PHQ-9 or GAD-7 that clients complete between reviews.

  • Can a 13-year-old start therapy in Illinois without a parent?

Yes, within limits. Under 405 ILCS 5/3-550, a minor aged 12 or older may request and receive outpatient counselling or psychotherapy without a parent's consent. For a minor under 17, services are first limited to eight 90-minute sessions. After that, they continue without a parent's consent only if the provider finds that seeking it would harm the young person's well-being, reviewed with a supervisor where possible, or if two attempts to reach a parent have failed and the minor gives written consent; if the minor will not allow a parent to be contacted otherwise, services end. Check the details with your counsel.

  • Does a HIPAA authorization satisfy Illinois's consent rules?

Not on its own. The Mental Health and Developmental Disabilities Confidentiality Act sets its own content for a written consent, including an expiry date and the right to revoke, and rules out blanket consent. HiBoop is HIPAA compliant and signs a BAA, but whether a particular disclosure is allowed is a question for your organization and its counsel.
