Measurement-Based Care for Nebraska Mental Health Teams
For certified community behavioral health clinics, regional network providers, integrated primary care and private practices from Omaha to Scottsbluff. Send validated scales by email or QR code, see them scored on submission, and chart each client's change over time, whether they live across town or three hours down the interstate. US customer data stays in Google Cloud US regions.
Nebraska's First Certified Community Behavioral Health Clinics
In May 2023 the Legislature passed LB 276, the Certified Community Behavioral Health Clinic Act, and told the Department of Health and Human Services to build a Medicaid prospective payment system for these clinics before 1 January 2026. On 24 September 2025 DHHS named the first seven providers to complete certification: CenterPointe, Community Alliance, Heartland Counseling Services, Heartland Family Services, Lutheran Family Services, South Central Behavioral Services and The Well. Their programs began operating in January 2026.
LB 276 requires each clinic to meet the federal certification criteria, or a state system substantially equivalent to them. The 2023 federal criteria from SAMHSA ask clinics to use standardized, validated and developmentally appropriate screening and assessment tools, encourage measurement-based care, and set out quality measures that include depression screening with a follow-up plan and depression remission at six months. The DHHS materials we read do not say which of those measures Nebraska will ask its clinics to report.
Medicaid already ties part of its managed care payments to quality. Under Neb. Rev. Stat. s.68-995, each Heritage Health plan holds back 1.5% of its at-risk capitation and earns it back on performance measures. For a care team, the practical step is a validated screen at intake and the same scales repeated through treatment, kept where you can count them. HiBoop scores the PHQ-9, the GAD-7 and the C-SSRS Screener on submission and charts them for each client. It does not report to DHHS or to a health plan for you.

What HiBoop Does for Nebraska Care Teams
Nebraska runs Medicaid through three statewide health plans, funds its public behavioural health system through six county-based regions, and is now certifying CCBHCs. HiBoop handles the measurement and leaves the clinical call with you.
Screening for CCBHC Intake
Send the PHQ-9, GAD-7 and C-SSRS Screener before a first appointment and read the scored results before you meet. A standard set at every intake gives your clinic one comparable baseline across programs and sites.
Repeat Scales, One Chart
Send the same scale at each visit or on a schedule and every new score lands on that client's chart. Remission becomes something your team can see and discuss, and the numbers inform your clinical judgement rather than replace it.
Check-Ins Without the Drive
Clients answer on their own phone from an email link, so a rancher in the Sandhills or a student in Chadron can complete a check-in between visits. It pairs with telehealth sessions and in-person care alike.
Exports for Health Plan and Regional Reporting
Export a client's score history as CSV or PDF for a treatment plan review, a records request or the measures your organization reports to a Heritage Health plan or its region. Your whole account can be exported whenever you choose.
Mental Health and Substance Use in One Library
Heritage Health plans cover physical health, behavioural health and pharmacy together. Depression, anxiety and substance use scales such as the ASSIST and CAGE-AID sit in one place for co-occurring caseloads.
Scales for Children and Teens
For pediatric and integrated primary care clinics, HiBoop includes the NICHQ Vanderbilt for attention and behaviour concerns, the SCARED for child anxiety and the PHQ-A for adolescent depression.

Research from Nebraska
Integrated pediatric behavioural health
In 2012-2013 electronic records from integrated pediatric behavioural health clinics in Nebraska, adolescents diagnosed with attention deficit and related disorders were more likely to be male, younger, publicly insured and living in rural or remote areas, while mood disorders were more likely among older, female and urban patients.

Depression among farmworkers
In a 2013 survey of 200 migrant farmworkers in rural Nebraska, 45.8% were classed as depressed, and the economics and logistics and health stress domains correlated with depression scores.
Ramos et al., Journal of Immigrant and Minority Health, 2015 PMID 25840517
Rural behavioural health workforce
Of 2,468 behavioural health professionals practising in Nebraska in 2012, 71.2% worked in metropolitan areas, 27.3% in rural areas and 1.5% in frontier areas.
Nguyen et al., Rural and Remote Health, 2016 PMID 27052101
Emergency department visits
Nebraska's state emergency department database recorded 52,035 visits for behavioural health disorders over 3 years, with US$96.4 million in charges, and rural regions had workforce shortages and high visit numbers at relatively few emergency departments.
Wani et al., Evidence-Based Mental Health, 2020 PMID 31907210
Telehealth for rural depression
In 10 focus groups across rural Nebraska, primary care physicians and patients they had treated for depression saw behavioural telehealth as a reasonable answer to access problems, while raising concerns about keeping therapeutic relationships at a distance.

Certified Community Behavioral Health Clinic Act
LB 276, 2023The Act adds CCBHCs to the Nebraska Behavioral Health Services Act. Clinics must offer outpatient mental health and substance use care, crisis services, screening and risk assessment, person-centred treatment planning and primary care screening, among other services. Medicaid pays them through a prospective payment system with daily or monthly rates, and the Legislature intended up to $4.5 million a year from the General Fund from 2025-26.

MedicaidSince 1 January 2024, Molina Healthcare of Nebraska, Nebraska Total Care and UnitedHealthcare Community Plan of Nebraska each serve members statewide, with physical health, behavioural health, dental and pharmacy in one plan. The 2023 quality strategy sets targets for follow-up after an emergency department visit for mental illness (64% within 30 days, 44% within 7) and asks plans to screen new members on depression, suicide risk and trauma.

Counties put in one dollar for every three dollars of state General Fund money. In fiscal year 2024 the Division of Behavioral Health spent $194 million on community-based services, including $65.6 million in community aid to the regions, and 23,166 people received those services.

Nebraska Mental Health Confidentiality and Consent
HIPAA sets the federal floor. Nebraska adds a confidentiality duty for licensed mental health practitioners, its own rules on patients' access to their records and a higher age of majority. None of these certifies software; the duties sit with your organization, and HiBoop's part is giving you what you need to meet them.
What Nebraska law asks
- Keep what a client tells a licensed mental health practitioner confidential unless the client consents in writing, the law or board rules allow disclosure, the duty to warn applies, or HIPAA permits it (Neb. Rev. Stat. s.38-2136)
- Give patients copies of their records on request, though a treating practitioner may withhold mental health records that, in their professional judgement, would not be in the patient's best interest to release, unless a court orders it (s.71-8403)
- Remember that majority is 19 in Nebraska, while a person aged 18 or older may consent to their own mental health services without a parent or guardian (s.43-2101)
- For Medicaid telehealth, get the client's informed consent before the first telehealth visit and every year after, and keep the written form in the record (471 NAC 1)

Frequently Asked Questions
Does Nebraska require measurement-based care?
Not as a general rule. We found no Nebraska statute or regulation that requires outpatient providers to use standardized symptom scales. The closest is LB 276: Nebraska's CCBHCs must meet the federal certification criteria or a substantially equivalent state system, and those criteria expect validated screening tools and encourage measurement-based care. Heritage Health plans also earn back held payments on quality measures. Check your own CCBHC agreement or health plan contract first.
Will HiBoop send our outcomes to DHHS, our region or a Heritage Health plan?
No. HiBoop does not connect to state, regional or health plan systems. It sends and scores the scales, charts each client's change, and exports score histories as CSV or PDF, so your team can prepare whatever its region or plan asks for.
Can an 18-year-old consent to therapy without a parent in Nebraska?
Yes. Majority in Nebraska is 19, but section 43-2101 lets a person aged 18 or older consent to their own mental health services without a parent or guardian. For younger clients, plan for parent or guardian consent and confirm the details with your counsel. Who sees a young client's scores follows your clinical policy, and HiBoop's role-based access lets you limit which staff can open them.
Is HiBoop HIPAA compliant for Nebraska practices?
Yes. HiBoop is HIPAA compliant, and a business associate agreement is available to covered entities. Nebraska's confidentiality statute for licensed mental health practitioners, section 38-2136, allows disclosure where HIPAA permits it, alongside written client consent and its other exceptions. Whether a given disclosure is allowed still depends on your situation, so check with your counsel.
