Measurement-Based Care for Indiana Mental Health Teams
For community mental health centres, CCBHC pilot sites, primary care practices and private clinicians from Gary to Evansville. Send validated scales by email or QR code, have them scored on submission, and see each client's PHQ-9 history on one chart as Indiana builds depression outcomes into its CCBHC metrics. US customer data stays in Google Cloud US regions.
Indiana Measures Need, and Now Depression Outcomes
Indiana has tied public behavioural health care to standardized measurement for years. Medicaid Rehabilitation Option services are available only to members who meet a level of need on the Child and Adolescent Needs and Strengths (CANS) or the Adult Needs and Strengths Assessment (ANSA), which providers enter in the Division of Mental Health and Addiction's DARMHA system. The state describes both tools as supporting care planning, level-of-care decisions, quality improvement and the monitoring of outcomes.
Symptom scores are now joining them. In its 2024 application for the federal CCBHC demonstration, Indiana wrote that the pilot sites had been submitting PHQ-9 scores and screening dates through DARMHA since July 2023. The certification criteria, updated in May 2025, require each certified community behavioral health clinic to collect and report depression remission at six months and depression screening with a follow-up plan, and to use at least one suicide risk assessment from a list that includes the C-SSRS. Measurement-based care itself is encouraged rather than required.
Those requirements apply to the CCBHCs, which began with eight pilot sites in January 2025, not to every provider in the state. For any team, though, the useful habit is the same: a standard scale at intake, the same scale repeated, and the history kept where it can be found. HiBoop does that for the PHQ-9, GAD-7 and C-SSRS Screener. It does not run the CANS or ANSA and does not connect to DARMHA.

What HiBoop Does for Indiana Care Teams
Indiana covers depression screening and brief behavioural assessment in Medicaid, divides community mental health care into designated service areas, and sets its own rules on consent and records. HiBoop runs the scales, scores and charts; your clinicians make the clinical calls.
PHQ-9 From Intake Onward
Send the PHQ-9 before the first visit and schedule repeats. Each score is dated and charted, so a clinic can see whether a client's depression is easing well before the six-month mark.
Suicide Risk Screening
The C-SSRS Screener is one of the suicide risk tools Indiana's CCBHC criteria accept, and it sits beside the PHQ-9 in HiBoop, so a positive item 9 can lead into a structured screen.
Records Behind Screening Claims
Indiana Medicaid covers 96127 and an annual depression screen under G0444, with validated tools such as the PHQ expected. HiBoop keeps the scale, score and date of every administration, exportable as CSV or PDF.
Anxiety and Trauma Alongside Depression
The GAD-7, PCL-5 and PC-PTSD-5 sit in the same library, so a co-occurring presentation is measured in one place and charted together.
For Centres With Many Clinics
Team and Network plans put a multi-county centre's clinicians in one account, with role-based access limiting each person to the clients their role allows.
Youth Screening
For adolescent caseloads in schools and primary care, HiBoop includes the PHQ-A, the SCARED and the CRAFFT-2.1, each scored on submission.

Adolescent depression screening
Of 2,038 young people aged 12 to 20 screened by computer at two Indianapolis primary care clinics, 303 (14.8%) screened positive on the PHQ-9, and referral to mental health services rose with severity from 34.2% (mild) to 72.2% (severe).
Aalsma et al., Journal of Adolescent Health, 2018 PMID 29174939
Electronic suicide screening
In the same two Indianapolis clinics, an electronic suicide screener in a decision support system found just over 6% of 2,134 patients aged 12 to 20 positive, and providers documented follow-up for 83% of them.
Etter et al., Journal of Adolescent Health, 2018 PMID 29195764
Collaborative depression care for older adults
In the Aging Brain Care Medical Home at Eskenazi Health in central Indiana, 66% of participants with PHQ-9 scores of 14 or more had at least a 50% reduction in depressive symptoms.

CCBHC Medicaid Demonstration
Since January 2025Indiana was named one of ten new demonstration states in June 2024, after Senate Enrolled Act 1 of 2023 provided $100 million for state fiscal year 2024 and $50 million for 2025. Eight pilot sites, including Centerstone of Indiana, Hamilton Center, Oaklawn and Sandra Eskenazi Mental Health Center, are paid a clinic-specific prospective rate outside managed care.

It has required the ANSA for adults served by these centres at admission, every six months and at discharge since 2009, and it runs DARMHA, the data system that holds those assessments.

Indiana Mental Health Records and Consent
HIPAA sets the federal floor. Indiana's code adds its own confidentiality rule for mental health records, a consent requirement before services begin, and telehealth standards. No state law certifies software; the obligations are your organization's, and HiBoop supplies safeguards that support them.
What Indiana law asks
- Treat a patient's mental health record as confidential and disclose it only with consent unless the code provides otherwise, using a written release that names the patient, the parties, the purpose and the information (IC 16-39-2-3 and 16-39-2-5)
- Obtain each patient's consent before providing mental health services (IC 16-36-1.5-4); a minor may generally consent only if emancipated or at least 14 and living independently, while a minor seeking substance use treatment from an approved facility may do so without parental consent (IC 16-36-1-3, IC 12-23-12-1)
- In telehealth, meet the in-person standard of care and, where a provider-patient relationship requires it, confirm the client's location and identity, disclose your licensure and obtain informed consent (IC 25-1-9.5-7)
- Keep HIPAA safeguards and breach notification, with a business associate agreement for every vendor; the Indiana Consumer Data Protection Act excludes HIPAA covered entities, business associates and nonprofits

Frequently Asked Questions
Does Indiana require measurement-based care?
Not for every provider. We found no Indiana statute or regulation requiring all mental health providers to use standardized symptom scales. Community mental health centres use the CANS or ANSA to establish Medicaid Rehabilitation Option eligibility, and CCBHCs must report depression remission at six months and depression screening. The CCBHC criteria encourage measurement-based care but do not require it.
Does Indiana Medicaid pay for 96127?
Yes, in fee-for-service: the Indiana Health Coverage Programs fee schedule updated in June 2026 lists 96127 at $4.03 from 1 January 2026, and G0444 covers an annual depression screen. On outpatient facility claims, 96127 is included in the revenue code 918 rate. The collaborative care codes 99492 to 99494, G2214 and 99484 are listed as not covered in fee-for-service; managed care entities set their own billing rules, so check with the plan.
Can a teenager in Indiana consent to therapy?
Indiana has no consent age specific to outpatient mental health care. Under the general rule in IC 16-36-1-3, a minor may consent if emancipated or if at least 14, self-supporting and living apart from their parents. A minor seeking treatment for alcohol or drug use at a DMHA-approved facility may do so without parental consent. Confirm specifics with your counsel; HiBoop's role-based access limits who sees a young client's scores.
Does HiBoop connect to DARMHA or complete the CANS?
No. HiBoop does not administer the CANS or ANSA and has no connection to DARMHA, Medicaid or plan systems. It sends, scores and charts validated symptom scales and exports each client's history as CSV or PDF for your own reporting.
