How mental health care works on the Island

Health PEI, a Crown corporation set up under the Health Services Act, runs the Island's public health services, mental health and addictions among them. Its 2021-22 annual report lists referring acute care hospitals, community hospitals, a psychiatric hospital and mental health and addictions sites, and counts 5,513 referrals to community mental health that year.

In November 2022 Health PEI signed an agreement with the Mental Health Commission of Canada, Stepped Care Solutions and a CIHR-funded team at Memorial University to bring in Stepped Care 2.0. The first step was One-at-a-Time counselling in its Community Mental Health and Addictions program. In stepped care, support moves up or down in intensity as a client's needs change, and a short score taken at each contact is one way a team can see when a step should change.

The province's ten-year plan, Moving Forward Together: Prince Edward Island's Mental Health and Addiction Strategy (2016-2026), sets five priorities. In its 2024 review, the Canadian Mental Health Association noted that the strategy does not say how progress will be measured.

What HiBoop Does for PEI Care Teams

  • The Health Information Act, stepped care and a small, spread-out population shape how outcome data is gathered on the Island. HiBoop handles the measuring; the clinical decisions stay with you.

  • Information Manager Terms in Writing

Section 42 of the Health Information Act requires a custodian to sign a written agreement with any information manager, protecting the information against risks such as unauthorized access, use, disclosure or destruction. HiBoop's published BAA policy gives its standard terms as an information manager, with the security documentation on our trust page behind them.

  • Each Role Sees Its Share

Section 39 asks a custodian to limit each authorized person to the health information they need for the purpose. In HiBoop, a front-desk coordinator, a counsellor and a clinical lead are each given access that fits their role.

  • Scores That Follow the Steps

When single sessions and longer courses of care run side by side, the same PHQ-9 or GAD-7 history stays with the client, so a decision to step care up or down is informed by what has actually changed.

  • A Record of Who Looked

Section 34 of the act recognizes a system that automatically logs who accessed health information, when, and what they could see. HiBoop records each view and export of client data, so you can review access whenever you need to.

  • Well-Being Alongside Symptoms

Teams can send the Satisfaction with Life Scale and the short form of the PANAS next to symptom scales. An Island-wide well-being survey in 2020 used both.

  • Read It in French

Health PEI offers some mental health services in both official languages, and the province has a bilingual health card. This page is in French too, for francophone staff and the clinics that serve them.

Research data

Research from Prince Edward Island

  • Antenatal psychosocial screening

In a randomized trial with 76 pregnant women seen by family physicians and public health nurses in three PEI health regions, both versions of the ALPHA psychosocial assessment form were acceptable to women and providers, and of the 238 issues disclosed, significantly more were raised with physicians than with nurses.

  • Student survey and feedback

Using SHAPES-PEI, a school data collection and feedback system, alongside New Brunswick's student wellness survey, a study of 3,318 PEI and 7,314 New Brunswick students in grades 7 to 12 found school connectedness positively associated with mental fitness and with each of its parts: autonomy, competence and relatedness.

Murnaghan et al., Journal of School Health, 2014 PMID 24889081

  • SWLS and PANAS

An online survey of 1,381 PEI adults in October and November 2020, using the Satisfaction with Life Scale, the PANAS and the WHOQOL-BREF, found lower well-being among LGBT, gender-diverse, Indigenous and disabled respondents and those living below the poverty line.

Aziz et al., International Journal of Community Well-Being, 2022 PMID 35729945

  • Peer mental health training

In an evaluation of the Resilient Minds program for volunteer firefighters on PEI, 84% of surveyed participants described the training as helpful, and at three months 83% could recall it.

The Health Information Act and Your Clients' Records

  • PEI's Health Information Act applies to custodians, a term that covers health care providers and the operators of health care facilities. The Information and Privacy Commissioner oversees it. The duties are yours as custodian; HiBoop's job is to give you what you need to carry them out.

  • What the act asks of a custodian

    • Keep each use and each disclosure to the minimum amount of personal health information the purpose needs (ss. 22 and 23)
    • Protect information with reasonable administrative, technical and physical safeguards, based on recognized security standards and taking account of where it is stored (s. 39)
    • Have a written agreement with any information manager that protects the information against unauthorized access, use, disclosure, destruction or alteration (s. 42)
    • Notify the individual and the Commissioner in writing, at the first reasonable opportunity, when information is stolen, lost or reached by someone without authority, unless no adverse impact is expected (s. 36)
    • Respond within 30 days when a client asks to see or copy their own record (s. 8)

Frequently Asked Questions

  • Is HiBoop compliant with PEI's Health Information Act?

The act places its duties on the custodian, so no software can meet them on your behalf. What HiBoop does is support you in meeting them: published information manager terms for your section 42 agreement, documented safeguards, access by role, and a log of every view and export.

  • Can client data be stored outside PEI?

Section 35 allows disclosure outside the province without consent only in the cases the act lists, and one of them is disclosure to an information manager under the act (s. 23(13)(f)). Section 39 also asks you to weigh where information is stored. HiBoop hosts Canadian customer data in Google Cloud Canada regions, which are not on the Island, so your agreement and privacy review should record that.

  • At what age can a young person consent on their own?

PEI law sets no fixed age. The Consent to Treatment and Health Care Directives Act presumes every person capable of consenting to treatment, and capacity depends on whether the person understands the relevant information and appreciates the likely consequences. The Health Information Act uses a similar test for consent to collecting and sharing health information (s. 14).

  • How does measurement fit with stepped care?

Health PEI's stepped care work began with One-at-a-Time counselling, where a client may come once or several times. A short scale at each contact gives the counsellor and the client a shared record of change, whichever step comes next.
