Can patients upload files or images in a form?

Yes. Your clinic's forms can include File and Image questions. Use them to collect supporting documents, like a photo of an ID or a referral letter.

Add an upload question to a form

1. Go to Settings > Forms and open the form you want to change.
2. Add a File or Image question to the form.
3. Save the form.

Upload questions can't go inside a container, list or grid. Add them directly to the form.

What can be uploaded?

• Image questions accept JPG, PNG and WebP images.
• File questions accept the same images, plus PDF files.
• Each question holds one file.
• Files can be up to 20 MB.

Who can upload?

Patients can upload a file when they fill in the form themselves. Staff can also upload one when they complete the form on a patient's behalf. The file is uploaded when the form is saved.

Where do I find uploaded files?

Open the patient's profile and click the Forms tab. Open the completed form to see the uploaded file, and click it to view it.
