You can issue an assessment to any patient—new or existing—right from their profile or during intake.
🆕 For New Patients
- Go to the Patients page
- Click “Add a Patient”
- Enter their details
- Select “Issue an Assessment”
👥 For Existing Patients
- Open the patient’s profile
- Click one of the available schedule types:
- Intake
- Follow-Up
- Discharge
- Create Custom Schedule

HiBoop supports three flexible options for delivering assessments:
Email: Sends a secure link directly to the patient
- Best for remote/virtual clients and follow-up assessments
In-Person (QR Code): Displays a scannable QR code
- Best for in-clinic use on phone or tablet
Clinician Portal: Clinician completes the assessment with or on behalf of the patient
- Best for assisted care or tech-limited clients

How do I make an assessment required?
Use Required Assessments when issuing an assessment. We recommend keeping the number of required assessments minimal and allowing the default recommendation algorithm to guide the process.
What information is available on the patient profile?
The Patient Profile includes:
- Demographic details
- Condition severity
- Assessment scores
- Individual assessment responses
How do I schedule an assessment for the future?
When issuing an assessment from the Patient Profile, select a future date in the Assessment Details section.
- If email is selected as the delivery method, the assessment will be sent automatically on that date.
- If in-person (QR code) or clinician portal is selected, the provider must administer the assessment manually.
A calendar feature will soon be available to help track upcoming assessments.

Once your clinic account is active, adding patients to HiBoop is quick and easy.
Steps to Add a Patient
- Navigate to the Patients page
- Click “Add a Patient”
- Enter:
- First name
- Last name
- Email address (used for assessment delivery)
4. (Optional) Proceed to Issue an Assessment immediately
- Select a delivery method - Email, in-person QR code, or clinician view
💡 Tip: Patients do not need to create an account. They’ll receive a secure link when assessments are delivered via email or QR code.

A schedule acts as a structured care plan for patient assessments across time.
🧭 Types of Schedules
- Intake – Initial assessment before treatment
- Follow-Up – Ongoing check-ins during care
- Discharge – End-of-treatment measurement
- Post-Care – Optional follow-up (e.g., 1 month later)
- Custom – Build your own schedule from scratch
Schedules help standardize care and simplify progress tracking across time points.

- In Assessment Details, select a future date
- Choose your delivery method:
- Email: Assessment is sent automatically on the selected date
- In-Person or Clinician Portal: Requires manual launch by provider

Yes! You can create programs to structure recurring assessments over time.

- Choose how many times the patient should be assessed
Example: 7x over 6 weeks (intake, 5 weekly check-ins, discharge, follow-up) - Define time intervals between assessments
Weekly, biweekly, custom dates - Select your preferred assessments and delivery methods
- Save and apply to patients as needed
What demographic information is collected?
HiBoop collects:
- Name & email
- Job Industry
- Substance use history
- Existing diagnoses
- Current medications
How do I schedule an assessment for the future?
When issuing an assessment from the Patient Profile, select a future date in the Assessment Details section.
- If email is selected as the delivery method, the assessment will be sent automatically on that date.
- If in-person (QR code) or clinician portal is selected, the provider must administer the assessment manually.
You can also create a Program for your patients:
- Select the number of times you would like to assess a patient over the duration of treatment. For example, for a 6-week program, you could assess 7x (intake, weekly check ins, discharge, follow up)
- Decide the amount of time between each assessment
- Pick your preferred assessments and delivery method
